PSYCHIATRIC NURSE PRACTITIONER invited to join a multi-site FQHC / community health center system in southeastern Vermont.

NEW GRADS WELCOME. In a certified Medical Home setting, you will become part of an established behavioral health team of over a dozen providers, and partner with primary care physicians and APPs to provide progressive services to support members of the community.

This group prioritizes patient-centered care and is accessible and affordable to all, based on a sliding fee scale. You will provide behavioral health consultations, make risk assessments, and manage short term interventions that include psychopharmacology stabilization.

You will consult with practitioners in affiliated health centers and interact with outside agencies and schools.
