Our innovative team nursing model pairs an RN with an LVN and therapists, and CHHA’s. We travel to patients' homes throughout the day.
As a Home Health Registered Nurse (RN), you will provide optimum care to our homebound patients in San Diego. You will be responsible for hands-on supervision for activities including medical necessity review, initial assessments, continued stay review, discharge planning, and other duties as required.
You will be expected to utilize all elements of the nursing process, assess, and evaluate patient / client status, and provide professional nursing care.

This position is a Pay per visit compensation model.
Pay per visit ranges from $50- $160 / commensurate with experience and depending on type of visit.

As an RN Case Manager, you will be responsible for traveling to patients' homes to provide care, completing and submitting accurate electronic clinical notes, administering medications and treatments as prescribed by the physician, and assisting patients with the activities of daily living.
You will also teach patients and families self-care techniques, provide medication, and diet instructions, and maintain a safe environment for the patient.
Qualifications for the position include :
- graduate of an approved school of professional nursing, with a BSN preferred.
- currently licensed to practice as an RN in the state of practice and
- clinical knowledge of various disease processes.
- one year of acute nursing experience in medical, surgical, and / or critical care. (Home health experience is preferred.)
- excellent observation, good nursing judgment, and good oral and written communication skills.
- self-directing, flexible, and cooperative in fulfilling their role obligations.
