The Dementia Caregiving Specialist 1 will collaborate with an interprofessional research team through an NIH-funded clinical trial to evaluate an innovative transitional care intervention for older adults with Alzheimer’s disease and related dementias (ADRD) discharged from a skilled nursing facility (SNF) and their caregivers.

The Dementia Caregiving Specialist will provide telephone-based support for caregivers of SNF patients with ADRD. After the discharge from the SNF, the Dementia Caregiving Specialist will independently call the caregiver at home, assist caregivers as they identify the barriers and facilitators to achieving the SNF discharge plan, and provide education and guidance to overcome barriers and engage facilitators.

Key care practices will be helping caregivers develop approaches to communicating with individuals with dementia and supporting caregivers as they fit the SNF discharge plan to the resources of the discharge setting and the occupational capacity of the patient with dementia.

In post-discharge support calls with caregivers, Dementia Caregiving Specialist 1 will support caregivers as they implement the discharge plan at home, assisted living, or nursing home long-term care setting.

Key responsibilities in providing post-discharge support will include :

1) Independent review of the SNF discharge plan and evaluation of post-discharge needs.

2) Independent review of caregiver progress implementing the SNF discharge plan and caregiver ability to implement the SNF while supporting the unique needs of the patient with ADRD.

3) Independent caregiver support will include care, such as identifying and resolving care needs (safety, symptom management, medications, coordination, and caregiver needs), adhering to study protocol to focus on the ADRD patient’s and caregiver’s specific, current concerns, educating caregivers to manage symptoms of ADRD, coordinating care to promote access and respond to emerging challenges, and providing support and community resources to manage caregiver strain.

Other activities will include documenting caregiver support calls, managing the call schedule, coordinating care with clinical staff in six nursing homes and a home health agency, and attending meetings with the research team.

Minimum Education and Experience Requirements Bachelor’s degree in a discipline related to the field assigned and one year of related training or experience;

or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.

Required Qualifications, Competencies, and Experience Degrees must be received from appropriately accredited institutions.

Additionally, dementia-specific training including micro-credential as a dementia caregiving specialist. Dementia-specific micro credential(s) must be received from a national organization Alz.

Assoc. AFA, NCCDP, PAC). Must have broad knowledge of dementia and dementia caregiving best practices. Must have demonstrated ability to consult with dementia caregivers across diverse situations, coordinate care, collaborate with teammates on projects, as well as to identify and solve problems independently.

The position requires significant attention to protocols and strong ability to balance multiple priorities and a complex timeline.

Must be able to work and communicate with diverse populations effectively and professionally.

Must be proficient with Microsoft Word and Outlook, Teams, and Zoom. Preferred Qualifications, Competencies, and Experience Prefer candidate with diverse clinical experience across the care continuum (hospital, SNF, home health, assisted living, palliative care, and / or hospice).

Prefer candidate with experience in phone-based consulting with dementia caregivers. Prefer candidate with experience teaching / training dementia caregivers.
