The Speech Language Pathologist is responsible for the evaluation and treatment of outpatients in compliance with licensure standards.
The Speech Language Pathologist works with an interdisciplinary team to identify patient goals, barriers to discharge and ongoing speech language services.
This position provides patient centered care and exercises independent judgement in planning, organizing and prioritizing workload.
Essential Job Functions :
Obtains and documents appropriately all required and pertinent patient information, including precautions, orders, and initial plan of care.
Orients patients, family and / or other appropriate parties to the purpose and procedures of the therapy evaluation and treatment.
Provides appropriate patient education, family teaching and support which relates to the needs of the patient and family.
Selects and performs evaluation and treatment methods which correlates to the age of the patient, the patient's diagnosis, symptoms and physician orders and documents per chart and review standards.
Coordinates treatment schedules with disciplines using appropriate scheduling procedures; participates with inter-disciplinary team in the development and accurate documentation of a discharge plan in the inpatient settings.
Completes patient’s charges according to treatment provided within the required timelines.
Identifies significant changes in patient's condition and takes appropriate actions, as evidenced in reporting, staffing consultations and documentation.
Monitors program effectiveness and outcomes, initiating programs and development of new programs.
Utilizes a team approach in the delivery and management of services and patient care.
Performs pain assessment and takes appropriate action according to standard operating procedure.
Performs other duties as assigned or needed to meet the needs of the department / organization.
Certifications :
Speech, Language, Pathologist (SLP) North Dakota State Board of Examiners on Audiology & Speech Language Pathology Prior to Start Date HR Primary Sources
Certificate of Clinical Competence in Speech-Language Pathologist (CCC-SLP) American Speech Language Associate (ASHA) Within 18 Months of Start Date HR Primary Sources
Basic Life Support (BLS) American Heart Association (AHA) Within 90 days of Start Date Learning Management System.
