Position Name : Registered Nurse Hospice - Urgent Priority Need
Location : Medford, OR 97502
Start Date : ASAP
Employment Type : Full-time, Travel, Contract, Temporary
Contract Length : 13 weeks-Accepting Local and Travel Candidates
Pay Rate : $2520 to $2600 Weekly
Unit : Home Health, Hospice
Shift : 8 Hours; Days, 8a 5p
Schedule : 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, No weekends.
Position Requirements :
- Registered Nurse : 2 years - Required
- Recent / Current Home Health Hospice Nursing experience : 1 year within the past 3 years - Required
- Home Care Home Base (HCHB) charting experience - Required
- Must have valid Driver's License and Personal Auto Insurance
- Must be Covid Vaccinated or provide medical or religious exemption
Education, License and Certification Requirements :
- ASN or an equivalent Required
- BSN or an equivalent Highly preferred
- RN License for State of Oregon - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description :
- Provide complete and comprehensive nursing care and end of life care for Hospice patients in a Home health setting.
- Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family / caregiver living situation to determine care
- Provides a complete physical assessment and history of current and previous illness(es).
- Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.
- Assesses and evaluates patient's status by establishing the initial individualized plan of care in collaboration with the certifying medical provider based on comprehensive assessment and patient goals.
- Regularly re-evaluating patient and family / caregiver needs.
- Participating in revising the plan of care as necessary in collaboration with the certifying medical provider as patient status and needs change.
- Administers medications and treatments as prescribed by the medical provider in the plan of care.
- Involves the patient and family / caregiver to address needs and meet related goals.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Facilitates the patient's efforts toward self-sufficiency or obtaining appropriate care giving.
- Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate patient care for assigned caseload.
- Provides bereavement support and end of life comfort as needed
- Territories : Medford and surrounding areas
