Job Description :
- Utilize an interpersonal process focusing on the total individual and role within family, culture, and society.
- Implement the nursing process, including assessment, diagnosis, planning, implementation, and evaluation.
- Advocate for residents by ensuring they have adequate information to make health decisions and by coordinating total care.
- Work with residents to achieve or maintain their highest practicable level of function.
- Ensure quality of resident care meets legal and professional standards.
- Act as a role model for other members of the health care team.
- Complete holistic, age-specific admission history, physical assessment, and nursing care plan on all assigned residents continuously.
- Analyze and incorporate clinical data to formulate an individualized care plan.
- Identify and document residents at risk for nutritional and functional status.
- Identify and report signs / symptoms of neglect / abuse related to child, domestic, or elder abuse.
- Set priorities and review the daily care plan in collaboration with resident, family, significant others, and other healthcare disciplines.
- Delegate to team members based on resident needs, continuity of care, and staff knowledge.
- Communicate with physicians to ensure coordination of medical and nursing care plans.
- Demonstrate competence in technical skills and proactively manage urgent / emergent resident care situations.
- Prepare and administer medications and IVs per scope of practice and hospital policy.
- Document nursing interventions, residents' responses to care, and resident outcomes.
- Evaluate data to revise the care plan to meet changing needs.
- Educate residents, families, and interdisciplinary team members on care needs, including discharge planning.
- Supervise, coach, counsel, and evaluate the performance of assigned CNA staff.
- Follow CMS regulations and Medicare skilled criteria to ensure documentation supports compliance and reflects resident status.
- Maintain organized, efficient delivery of care by a team assigned for daily tasks and delegate appropriately.
Skills & Experience :
- Ability to read and write at a level sufficient to fulfill documentation requirements.
- Proficiency in technical skills such as typing, filing, and utilization of content management systems.
- Physical capability to lift objects over 100 lbs, with frequent lifting / carrying up to 50 lbs.
- Effective communication and collaboration skills for working with a diverse team and resident population.
Preferred Skills & Experience :
- Understanding of discharge planning, patient education, wound care, and X-ray reading.
