Permanent and full-time, in-person position.
QUALIFICATIONS / REQUIREMENTS :
- B.S. Pharmacy, Pharm.D. preferred
- License or eligible for license as a Registered Pharmacist in Connecticut
- Inpatient hospital pharmacy work experience preferred
- Post-graduate pharmacy residency or fellowship training preferred
Major Accountabilities / Critical Responsibilities :
- Prepare and dispense medication accurately following protocols.
- Provide timely drug information to healthcare professionals and patients.
- Conduct medication counseling sessions for patients.
- Deliver quality pharmaceutical care to all patients.
- Ensure therapy appropriateness for all patient demographics.
- Monitor patient therapy according to departmental policies.
- Identify and address medication-related issues promptly.
- Manage drug distribution and controlled substances.
- Supervise pharmacy support personnel.
- Complete required training and education.
- Ensure cost-effective medication therapies.
- Assist with pharmacokinetic consult services.
- Follow therapeutic substitution policies.
- Participate in quality improvement activities.
- Contribute to hospital initiatives and committees.
- Adhere to 340B program requirements.
- Demonstrate fiscal responsibility.
- Follow regulatory standards.
- Stay updated on drug use through continuing education.
- Precept pharmacy students when assigned.
- Provide drug information in-services as needed.
- Contribute to quality initiatives as assigned.
- Work flexible shifts as needed.
- Communicate effectively with healthcare professionals.
