Able Home Care is looking for Home Health Care Occupational Therapist, Per Diem and weekend opportunities. We have immediate needs in the North and South Shore areas.

Responsibilities :
- Provides services in accordance with organization's accepted standards of practice
- Completes initial and ongoing assessments to determine level of functioning
- Develops and revises the plan of care in consultation with the physician and other team members
- Establishes realistic measurable goals with patient
- Provides therapeutic treatments
- Conducts ongoing medical documentation, writing OASIS assessments as appropriate. Initiates appropriate preventive and rehabilitative nursing procedures, making necessary referrals to community services.
Prepares clinical and progress notes, submitting all required paperwork daily. This is non-negotiable.
Coordinates services, informing the physician, management, and other personnel of changes in the client's condition and needs.
Ensures quality and accuracy of patient records and charts in compliance with Medicare and Mass Health standards.
Responds promptly to pages, emails and phone calls.
