Aventura at the Bay is a 274 bed Skilled Nursing Facility located in St. Petersburg, Florida. We are currently seeking a Social Services Assistant / Social Worker that is energetic, motivated, and talented to join our team!
As the Social Worker you will assist the Director of Social Services in the ongoing efforts to facilitate and improve each resident’s social, emotional and psychological well-being.

Job Summary & Qualifications :
Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident.
Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
Work with residents, providers and families to discharge residents appropriately or discuss long term care.
Work with residents on grievances and concerns and follow-up.
Issue NOMNC per physician and therapy orders.
Work with staff on resident care plans.
Collect data as it relates to the MDS and Care Plans for new admits
Assist residents and families with adjustment to changes, including room changes.
Document progress notes related to care plans.
Interview residents / families as necessary and in a private setting.
Make routine visits to residents and perform services as necessary.
Minimum (1) year experience in social work, preferably in an LTC / SNF.
Ability to communicate at all levels of organization.
Polished Communicator, Customer Service Oriented.
Ability to work with minimal supervision, take initiative and make independent decisions.
Approachable, flexible and adaptable to change.
