Adelphi is seeking a Licensed Clinical Social Worker in Lane County, Oregon to conduct clinical assessments and provide therapeutic services to individuals and families, assist in the coordination of care and connect clients with community resources and participate in team meetings and contribute to case discussions.

Requirements :

• Must have valid Oregon state license.
• Master’s degree in social work with appropriate licensure.
• Must have current LCSW licensure in the state of Oregon.
• Relevant clinical experience in behavioral health.
• Know how to navigate EPIC / OCHIN Electronic Health Record system.
• Familiarity with community health practices and integrated care models preferred.

Responsibilities :

• Conduct assessments to evaluate clients’ mental health and social service needs.
• Provide individual, family, and group therapy sessions.
• Develop and implement treatment plans based on client assessments.
• Advocate for clients’ needs and connect them with appropriate community resources.
• Monitor and document client progress and treatment outcomes.
• Collaborate with other healthcare professionals to coordinate care.
• Participate in team meetings, case conferences, and supervision.
• Stay current with social work best practices and continuing education requirements.
