SUMMARY : Under the general supervision of the Manager, Clinical Services, is responsible for Client Development Evaluation Report of Diagnostic Elements (CDER).
Has no supervisory responsibilities.

$31.5543 - $42.2857 per hour

ESSENTIAL DUTIES AND RESPONSIBILITIES :
Review Coding on Client Development Evaluation Report (CDER) of Diagnostic Elements and code up to six (6) medical conditions as appropriate.
Correct all duplicate diagnosis in the system by working with case control.
- Update all CDER of clients seen via Clinical Review weekly.
- Review consumer's medical records including specialist reports.
- Identify chronic major medical diagnosis.
- Translate medical diagnostic terms into the most updated International Classification of Diseases (ICD) codes, and utilize insurance codes when necessary to identify coverage and for statistical information.
- Use the most current ICD to update the CDER. Remove medical conditions that are resolved.
- Discuss challenging cases with IRC consulting physicians. Follow up with community physicians regarding risks related to procedures, consents or other items needed to back up the Clinical Nurse.
- Keep daily log of completed CDER's coded and submit monthly to the Clinical department's Consumer Support Technician (CST) for processing.
- When requested, attend Clinical Review and give input on medically related issues and consultation as part of the Clinical Team.
- Provide back up to the Clinical Nurse as needed and handle emergency requests consents as appropriate when the Clinical Nurse is not available.
- Identify resources and necessary labs to help potential Genetic Clinic.
- Complete Nursing Assessments in coordination with Quality Assurance unit or other units as requested.
- Seek to maintain current and to expand relevant knowledge base. Attend training sessions as required.
- Develop trainings on important medical care topics as assigned and provide in-service staff training through unit meetings or in coordination with the training unit.
- Help complete In-Home Supportive Services (IHSS), Supplemental Security Income (SSI), Americans with Disability Act (ADA), protective supervision and other forms for clients when the client / family has no other source of help completing these forms.
- Use office equipment appropriately and keep it in good repair. Keep work area neat and orderly. Observe all safety rules.
- Work cooperatively and effectively with others. Perform as a member of the team, answer questions, share expertise and contribute to the harmony of the team.
- Handle change well and be flexible and adaptable in dealing with interruptions, new priorities and new assignments.
- Maintain good attendance and punctuality.
- Keep manager informed of pending work, work in progress and problems encountered.
- Utilize agency's IT systems as assigned, maintaining security and following agency's protocol and procedures, rules, and requirements.
- Attend training sessions as required.
- Comply with and ensure program compliance with agency's Personnel Policies and Procedures.
- Perform different or additional work as assigned.
MINIMUM POSITION REQUIREMENTS :
- Bachelor of Science Degree in Nursing.
- Five (5) years of nursing experience.
- Current California Registered Board of Nursing License
- A Public Health Nurse (PHN) Certification is preferred; however, applicants who agree to obtain the certification within 12 months of hire are also eligible to apply.
- Bilingual preferred.
- Ability to handle change well and be flexible and adaptable in dealing with interruptions, new priorities, and new assignments.
- Ability to follow oral and written direction.
- Ability to establish and maintain effective working relationships with others.
- Good verbal and written communication skills.
- Full use of an automobile, possession of a valid California driver's license and liability insurance for the minimum amount prescribed by law or ability to provide for independent transportation.
Must have and maintain a safe driving record.
