Open Door is seeking a Full-time bilingual English-Spanish speaking Licensed Clinical Social Worker for its Family Medical Center in Port Chester, NY.
The Licensed Clinical Social Worker will report to the Director of Behavioral Health, based in Ossining, NY. The dedicated individual will work closely with the primary health care team to provide integrated behavioral health care to improve the patients’ medical outcomes.
The licensed clinician will possess excellent clinical skills, provide short term Behavioral Health care using evidenced-based treatment models with adults, children, and families, and establish and implement clinical treatment goals in collaboration with the integrated health care team.
Responsibilities :
- Applies high quality, appropriate short term treatment based on clinical diagnosis and assessment of the patient’s presenting problem.
- Provides short term treatment modalities at Open Door site including, but not limited to, individual, family, and group counseling.
- Accurately documents all clinical interventions, diagnosis, symptoms, presenting problems in the Electronic Medical Record.
- Works with patient to develop treatment plan and goals, based on social, emotional, and health needs of the patient / family.
- Provides crisis intervention and conducts Warm Hand Off assessments as needed.
- Incorporates health and wellness education and referrals to preventive care.
- Coordinates and case manages plans within agency and with outside agencies, including interfacing with school, criminal justice, medical, and social service agencies.
- Coordinates care with patient’s care team, and communicates regularly with care team.
- Participates in weekly one hour supervision with LCSW supervisor.
- Participates in multi-discipline team conferences and service training programs.
Requirements :
- Master’s Degree in Social Work
- Current New York State registration and license as a Licensed Clinical Social Worker (LCSW)
- Bilingual Spanish-English
- Minimum of 3 years of experience providing clinical treatment to families and children, preferably in the health care field, in individual, family, group, and crisis intervention modalities
- Excellent clinical experience with mentally ill, substance abuse afflicted population
- Excellent clinical, assessment, and diagnostic skills
- Proficient in the use of computers, and specifically with electronic medical records
- Excellent written and verbal communication skills with a culturally diverse population
- Team-focused collaborator

- Passionate about providing high quality care to low-income, underserved residents in healthcare setting
- Highly organized, self directed, and able to multitask
