Healthcare Staffing Professionals has an immediate need for a Medical Case Worker, Navigator (Program : Division of HIV / STD).
The Medical Case Worker will under professional direction, function as a member of a medical or psychiatric multidisciplinary team, providing services to patients and clients in hospitals, clinics, rehabilitation, residential, or health care facilities.
This specific position is responsible for interviewing clients, discharge planning, linkage to medical services, navigation support in the field, and collaborating with other key partners.
The position will provide services in multiple locations including clinic locations, field offices and jails.
Position will start upon acceptance and employment clearance and is initially funded through 6 / 30 / 2025
Pay Rate
$28.71 / hr
Worksite Koreatown and field locations including jails and clinics

Minimum Qualifications :
- One year of experience providing services to patients and clients in hospitals, clinics, rehabilitation, residential, or health care facilities. -OR-
- Bachelor's degree from an accredited college or university and one year of professional casework experience interviewing, counseling, and assisting patients / clients with social problems.
- A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
