We're thrilled to announce a dynamic opportunity for you to become an integral part of our Houston, Texas team. As an onsite contributor, you'll play a key role in pioneering a pathway toward a more streamlined and effective healthcare system.

JOB SUMMARY :

The Care Coach, a nationally certified medical assistant, engages directly with patients through one-on-one encounters and group sessions.

Their primary role is to support patients in managing their chronic conditions and achieving their established care plan goals.

DUTIES AND RESPONSIBILITIES :

  • Conduct face-to-face visits with patients to promote and reinforce established care plan goals, including education on lifestyle modifications.
  • Collaborate with the Care Educator Nurse to discuss cases and ensure the closure of patient care gaps.
  • Identify and attend to same-day patients who can benefit from participation in the care education program.
  • Facilitate patient group workshops as coordinated by the Care Program Nurse Manager.
  • Accurately document patient information within the Electronic Medical Record (EMR) system.
  • Participate in health fairs and other events aimed at promoting health within the community.
  • Fulfill any additional duties assigned by the Care Program Nurse Manager.

Requirements :

  • National certification as a medical assistant.
  • CPR / BLS required.
  • Two years of medical assistant experience preferred.
  • Bilingual (fluency in speaking Spanish) is preferred.
  • Strong interpersonal skills and ability to engage effectively with patients.
  • Knowledge of chronic condition management and lifestyle modification techniques.
  • Proficiency in documenting patient information in an EMR system. Epic experience highly preferred.
  • Ability to collaborate effectively within a multidisciplinary team.
  • Flexibility to adapt to changing priorities and patient needs.
