Role Description
This is a full-time on-site role for a Case Manager at 1776 Senior Care in Glen Ellyn, IL. The Case Manager will be responsible for day-to-day tasks related to coordinating and managing care for seniors.
This includes assessing client needs, developing care plans, coordinating services, and communicating with families and caregivers.
Qualifications
- Strong organizational and multitasking skills
- Excellent communication and interpersonal skills
- Experience in case management or a related field
- Knowledge of senior care and geriatric issues
- Ability to work effectively in a team
- Empathy and compassion for seniors and their families
- Strong problem-solving and decision-making abilities
- Proficiency in relevant computer software and tools
- Bachelor's degree in social work, nursing, or a related field (preferred)
