Marion County Adult Behavioral Health is hiring an Exceptional Needs Care

Coordinator. This position provides services to individuals with serious and

persistent mental illnesses and complex needs, many of whom also have co-

occurring substance use disorders and medical conditions. You would have the

opportunity to build strong relationships while liaising with community and

state partners, work as a member of a highly skilled multidisciplinary

treatment team, and support individuals to achieve their highest level of

health. You will not only collaborate with a wide range of individuals from

diverse backgrounds but also have numerous opportunities to engage with and

address systems-level challenges. An ideal candidate would be compassionate,

highly collaborative, have strong clinical skills, and excellent written and

LCSW licensure supervision.

We offer the following recruitment bonus, prorated based on FTE :

$2,000 bonus for employees new to Marion County starting on January 1, 2024, through June 30, 2025. Bonus will be included in employees first paycheck.

For those relocating from outside of Oregon, we provide the following

relocation package :

  • Up to $3,000 reimbursement of moving expenses
  • Relocation bonus in the amount of $3,000 (prorated based on FTE) as a one-time payment upon proof of Oregon residency.

A wage enhancement of $3.00 will be added to the hourly wage as compensation

for hours worked between 6 : 00 p.m. and 6 : 00 a.m.

GENERAL STATEMENT OF DUTIES

Provide assessments and treatment of individuals and families, including

groups; provide crisis assessment and intervention services and related duties

as required. The primary responsibility of this class is to provide assessment

and treatment services, crisis intervention, and treatment services, including

intensive services, to individuals and their families with complex needs and

diagnoses or to develop and implement specific services in the community.

SUPERVISION RECEIVED

Work under a supervisor who assigns work establishes goals and reviews the

results for overall effectiveness.

SUPERVISION EXERCISED

Primary supervision of employees is not a responsibility of this

classification; however, positions may act as lead workers as defined in the

Marion County Personnel Rules.

Typical Duties - Duties include, but are not limited to the following

The Choice Model Exceptional Needs Care Coordinator (ENCC) provides services

to individuals with serious and persistent mental illnesses and complex needs,

many of whom also have co-occurring substance use disorders and medical

conditions. This position is an outward facing position, operates on a systems

level and involves coordination with entities on the state and local levels,

and involves making sound clinical decisions with complex cases.

Exceptional Needs Care Coordination

  • Act as a liaison (with supervisory oversight) between ABH and various stakeholders involved in mental health care, including Oregon State Hospital (OSH), acute care hospitals, coordinated care organizations (CCO), Aging and People with Disabilities (APD), Intellectual and Developmental Disabilities Services (IDD), other programs within Marion County Health & Human Services, etc.
  • Collaborate with individuals in the mental health system, partners, and stakeholders to assess individuals needs, determine eligibility for services, appropriateness of services, authorize enrollment for Choice Model services, etc.
  • Attend OSH Interdisciplinary Treatment Team (IDT) meetings for individuals.
  • Participate in Ready to Transition determination with OSH.
  • Facilitate community transition for individuals who fall under Choice criteria.
  • Provide case specific consultation to providers and community partners, including clinical recommendations for treatment, services, and risk reduction.
  • Collaborate with acute care staff regarding care recommendations, diversion opportunities, and Long-Term Psychiatric Care needs.

With clinical supervisor acting as CMHP delegate, initiate authorization process for requests for Long-Term Psychiatric Care.

  • Ensure accuracy of utilization data of those admitted to the Oregon State Hospital and acute care hospitals, provide routine reports to Marion County Management.
  • May require overnight travel to participate in coordination meetings for individuals deemed Marion County of Responsibility placed in other counties.

Licensed CareResidential Coordination

  • Assess individuals needs, determine eligibility for services, appropriateness of services, and authorize licensed careresidential placements.
  • Identify and coordinate placements in appropriate licensed care settings, within and outside of the mental health system, for individuals discharging from OSH, acute care hospitals, andor residing in the community.

With supervisory support and oversight, negotiate with Oregon Health Authority (OHA) to advocate for Rate Reviews for complex cases.

  • Consult on LOCUSLSIPCSP assessments utilized in rate setting for licensed providers as needed.
  • Organize, facilitate, andor attend treatment planning meetings for individuals enrolled in Choice Model services and collaborate with providers and agencies involved in individuals care.

Delegate such tasks to ENCC QMHA as needed.

  • Make recommendations to ABH leadership for funding for Choice Model services for licensed care placements, Supportive Housing, and other needs as identified that interfere with placement.
  • Assess individuals readiness to step down to lower levels of careplacement when identified.
  • Ensure residential capacity, track referrals, placements, placementdischarge dates, and utilization data is being tracked by Choice team.
  • Provide clinical consultation in response to complex cases, critical andor unusual incidents, grievances, and complaints within the residential system;

this includes bi-annual training to providers on topics related to mental health.

Assessment & Treatment Planning

  • Complete intake and annual comprehensive mental health assessments, develop individualized treatment plans, develop crisis prevention plans, and direct provision of treatment services.
  • Provide therapeutic intervention and services to individuals, including crisis assessment and intervention.

Documentation

  • Maintain accurate and timely clinical documentation in accordance with federal, state, county, and treatment program requirements under the direction of the clinical supervisor.
  • Meet performance expectations for time and billing standards.
  • Compile and submit Choice Model reports to the Oregon Health Authority.

Other Duties

  • Participate in request for proposal process.
  • Participate fully as a member of the Marion County Adult Behavioral Health team as a supportive, reliable, positive, collaborative, and professional team member.
  • Work with a team of clinicians and supervisor for clinical supervision, treatment planning, staff, and departmental meetings, and trainings.

Response Level 2 : In the event of an emergency, employee makes a reasonable

effort to report to work. Employee may be required to perform duties outside

of their regularly assigned duties. Employee may be requested to work an

extended or flexible schedule. Staff at this response level must complete

Incident Command System 100 and 700 on-line or in-person training.

Requirements for the Position

EXPERIENCE AND TRAINING

  • Masters degree in psychology, social work, recreational therapy, music therapy, art therapy or a behavioral science field; OR
  • Bachelors degree in nursing, or occupational therapy and licensed by the State of Oregon; AND
  • One year of postgraduate degree experience in the behavioral health field.

PREFERENCES

Preference will be awarded to candidates who are LCSW, CSWA or eligible for LCSW. Qualified candidates will be eligible for licensure supervision if hired.

SPECIAL REQUIREMENTS

Must be either 1) licensed by the Oregon Board of Licensed Professional Counselors, 2) licensed by the Oregon Board of Licensed Social Workers or 3) credentialed or registered through the Mental Health and Addiction Counseling Board of Oregon (MHACBO) as a Qualified Mental Health Professional (QMHP) within the first 30 days of employment.

Must possess a current drivers license in the applicants state of residence and an acceptable driving history. Marion County will obtain a copy of the driving record for all qualified applicants from Driver and Motor Vehicle Services and review the driving record according to the Marion County policy and procedure for Driving on County Business.
