Thisposition will be covering the Forsyth / CherokeeCounties.
A s a Home Health LPN , youwill :
- Provide professional nursingservices under the direction of a RN in compliance with the state'sNurse Practice Act, any applicable licensure / certificationrequirement, applicable healthcare standards, governmental laws andregulations, and CenterWell Home Health policies and procedures.
- Provide skilled nursing interventions in thetreatment of the patient / clients illness, rehabilitative needs andpreventative care.
Utilize a holistic approach in the provision ofproblem specific therapeutic interventions, teaching and trainingactivities in accordance with the plan of care.
- Apply knowledge and skills in accordance withaccepted standards of clinical practice to facilitate problemresolution and achieve individualized patient goals andoutcomes.
- Confer with the RN Supervisorregarding needed changes in the Plan of Treatment. Accept verbalorders from physician were permitted by state law / regulations / NursePractice Act and communicate these orders to the RN Supervisor.
- Utilize a systematic, individualizedgoal / outcome driven approach in implementing the nursing plan ofcare.
- Maintain contact with patients,physicians, clinical manager(s), other members of the healthcareteam in a timely manner regarding patient / family needs and statuschanges.
Participate in care coordination activities and dischargeplanning as appropriate.
- Maintain the higheststandards of professional conduct in relation to information thatis confidential in nature. Share information only when therecipient's right to access is clearly established and the sharingof such information is dearly in the best interests of the patient.
- Appropriately communicate to ensure adherenceto professional standards in the provision of and availability ofsupplies, materials and equipment needed to safely and effectivelyimplement the plan of care.
- Prepare, submit andmaintain documentation as required by the Company and / or facility.Visit / shift notes documented on day services arerendered.

Required Experience / Skills :
- Graduate of an accreditedLicensed Practical Nursing Program or accredited School ofVocational Nursing.
- Current nursing license inthe practicing state.
- Valid drivers license,auto insurance and reliable transportation.
- Current CPRcertification.
- Two years experience as anLPN / LVN in a clinical setting, preferably in a home health orhospicesetting.

The compensation range below reflects agood faith estimate of starting base pay for full time (40 hoursper week) employment at the time of posting.
The pay range may behigher or lower based on geographic location and individual paywill vary based on demonstrated job related skills, knowledge,experience, education, certifications, etc.
