Key Responsibilities :
- Conduct comprehensive assessments for prospective residents, focusing on nursing needs and suitable placement.
- Identify Resource Utilization Groups (RUGs) and associated costs for potential admissions.
- Review and assess applications for admission via on-site or phone evaluations, coordinating closely with the Admissions team.
- Ensure accuracy and compliance in the MDS / PDPM process for all residents.
- Stay abreast of Medicare criteria, serving as a key resource for nursing staff and communicating regulatory updates.
- Regularly create and update assessments for both skilled and long-term residents, including weekly, quarterly, and monthly evaluations.
- Oversee the Case Mix Index (CMI), identifying any potential risks or changes affecting Medicaid reimbursements.
- Lead problem-solving for complex admissions and monitor Medicare assessment schedules to ensure precision and timeliness.
- Prepare and present accurate census data and information at meetings, and actively participate in developing care plans during team meetings.
- Manage and oversee CNA documentation related to MDS, including ADLs, bath sheets, and other assessments.
- Provide ongoing education and training for staff, ensuring a high standard of care.
- Supervise and manage nursing operations and staff for your assigned unit, including participating in the hiring and training process.
- Conduct regular chart and MAR audits to maintain the highest standards of care.
Qualifications :
- Must be a Registered Nurse (RN).
- Prior experience as an MDS Coordinator is preferred but not essential.
- Previous experience in a nursing home or similar setting is advantageous.
