Job Title : Specialist-Nurse Ops / Nurse Assessors
Location : New York, NY
Job Summary :
- Review health status questions and make evaluations based on UAS criteria for enrollment.
- Utilize technology in the workplace.
- Apply knowledge of general professional nursing principles and practices.
- Comply with state and agency laws and regulations governing nursing practices.
- Demonstrate strong organizational, interpersonal, written, and verbal communication skills.

Job Responsibilities :
- Perform initial in-home pre-evaluations of patients, evaluating health status, strengths, care needs, and preferences to develop individualized long-term care service plans.
- Review consumer medical documentation and health referral forms.
- Enter evaluation data into electronic evaluation forms and transmit as required.
- Document concerns, conflicting information, or issues during the evaluation process.
- Promote continuity of care to reduce fragmentation and gaps in treatment plans.
- Discuss health care options, supports, service vendor options, and waiver options with consumers.
- Initiate communication between the Call Center and eligible consumers for managed care health plan enrollment.
- Enhance communication and relationships with interdisciplinary care team members to improve care coordination.
- Collect quality review data and required documentation for outcome measurements.
- Maintain comprehensive knowledge of community resources, payor requirements, and network services.
- Meet established standards and performance criteria.
- Perform other duties as assigned by the Regional Nurse Manager or other project management.
Qualifications :
- Required Experience : Two (2) years of home setting experience within the last ten (10) years, including various types of experience.
- Recent experience conducting comprehensive assessments of patients.
- Current RN Licensure in New York State.
Working Hours :
- Full-time nurses work 4 days with specific start times (8 : 30 am, 9 : 00 am, 9 : 30 am, or 10 : 00 am Eastern Time).
- Part-time nurses commit to a minimum of 5 assessments per week on flexible days.
- Full-time training for 13 days from 8 : 30 am to 5 : 00 pm Eastern time.
- After training, availability during the week for precepting / shadowing for 2 weeks (preceptors do not work on weekends or after 5 : 30 pm).
