ElderChoice is looking for an energetic individual to join the local New Hartford team of home health care professionals!!
The Regional Field Support Staff will act as a support to the Personal Care Aides in the field, conduct home visits to ensure quality of services.
Successful candidate must be a current Personal Care Aide or Home Health Aide OR meet the qualifications and complete training to become a PCA, through ElderChoice.
Travel in and around Onieda County, Herkimer County, and Madison County, at a variety of times / days, is mandatory.
Job Summary :
The primary purpose of the Regional Field Support Staff (RFSS) is to provide assistance to Direct Care Staff (DCS) in the field and to the members of the Direct Care Management Team.
The RFSS will also ensure that strong communication exists with the Direct Care Management team and will assist the RN and DCM team in ensuring that the patient’s Care Plan is being implemented as written.
Essential Job Duties and Responsibilities :
- Provide support and oversight to DCS in the patient’s homes.
- Observe the patient environment for safety.
- Ensure that home binders are present and complete.
- Ensure that DCS are following the dress code and have their agency badge.
- Ensure that the DCS are aware of the patient’s goals and that the care plan is being followed.
- Complete on shift visits for DCS to provide support, to follow up on concerns and / or as a result of a recommendation from an agency investigation or per ElderChoice Policy
- Provide ongoing coaching to DCS related to documentation, patient satisfaction, patient specific information, and other related items.
- Deliver PPE’s, care plans, etc to patient homes as requested by the DCM.
- Provide support and oversight to DCS via phone calls.
- Assist the DCM with the scheduling needs of patients and DCS.
- Manage patient and staff MediSked schedules in an accurate and real-time basis; adjusting schedules as needed.
- Ensure staff, patients, and other required parties are informed of changes as required.
- Document communication with patients, families, Service Coordinators follow-up via Patient Notes and Staff Progress Notes to show clear and consistent communication and follow up to issues.
- Assist the DCM with reminding assigned DCS of all on-going annual requirements including : PPDs, Physical Assessments, Annual In-Service and trainings as directed by DCM.
- As a member of the Direct Care Management Team, perform emergency fill-ins, as deemed necessary to ensure the health and well-being of the patient.
- Attend team meetings as requested by DCM.
Minimum Qualifications :
- Must be a Certified Personal Care Aide, as certified by the NYS DOH or Dept of Education, or have completed Alternative Competency Demonstration testing.
- Be at least eighteen years of age
- Be able to read and write, as well as the skills necessary to follow written and verbal instructions required to meet the patient’s needs, as addressed through this service
- Be able to pass all required background, pre-employment checks and physical conditions required of this position, including, but not limited to :
