Our company is seeking a compassionate and experienced Nurse Practitioner to join our Advanced Illness Management team. This role involves providing advanced illness management and palliative care services to patients in various locations across the state.
Responsibilities include developing treatment plans, conducting consultations, and ensuring comprehensive care from the comfort of patients' homes.
The position requires up to 80-90% statewide travel, with potential for some occasional regional travel as well. This is an excellent opportunity for a dedicated healthcare professional to make a meaningful impact on the lives of patients and their families.
- Requirements :
- Master of Science in Nursing
- 2+ years Nurse Practitioner experience
- Active and unrestricted Nurse Practitioner certification / license
- Multi-state licensure is desirable
- ACHPN certification in Hospice and Palliative Care preferred
- Travel : Up to 80-90% (by car)
