Sentara Medical Group has a full-time opening for a Medical Director to join our Sentara PACE (Program for All-inclusive Care for the Elderly) team located in Norfolk, VA .
We are looking for a Family Medicine / Internal Medicine Physician to lead an interdisciplinary team to manage a small panel of 150-500 elderly patients, focused on quality and clinical outcomes.
We have two innovative practice models, our Norfolk Comprehensive Care Clinic and our Norfolk PACE program. The ideal candidate will have a background in value-based care.
PACE is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community.
It is an alternative to nursing facility eligible individuals who would prefer to remain at home with services coordinated by Sentara PACE.
The program provides supportive care for participants, including comprehensive medical and rehabilitative services, in-home services and transportation.
Position Highlights :
