• Required Certifications : BLS, LCSW, MSW, State licensed
• Required Skills : Coordinates with internal and external team members, Documents pertinent information to facilitate care coordination

Job Details :

• Pay : $2408.88 / week - $2634.22 / week
• Assignment Length : 13-week assignment
• Shift : 5, 8-Hour 08 : 00 - 16 : 30
• At least one year of experience required
