About The Position
Job Title : Rehabilitation Therapy Professional Job Specialty : Physical Therapist Job Duration : 13 weeks Shift : Monday - Friday Guaranteed Hours : 32 hours per week Experience : New Graduates Welcome;
SNF Experience Preferred License : State License Required Certifications : N / A Must-Have : - Covid test within 72 hours prior to start date with results available 48 hours before starting - Prescreen form completed and attached Job Description : - Provide physical therapy services to patients in accordance with their treatment plans - Conduct patient evaluations and formulate individualized treatment objectives - Collaborate with medical professionals and caregivers to assess patient progress - Document patient progress and modify treatment plans as necessary - Maintain compliance with federal, state, and professional regulations - Engage with interdisciplinary team members for comprehensive patient care - Demonstrate excellent communication and interpersonal skills Please ensure all required documents including a resume and license verification are attached prior to submission.

Credentialing Checklist : - State License Verification - Three References - County Criminal Check (7 years for all counties lived and worked)- Federal Criminal Check (7 years or more)- Multi-State Criminal Search (7 years or more)- Nationwide Sex Offender Search (7 years or more)- HHS / OIG Search- GSA Search- Social Security Number Trace- 2-Step PPD or Quantiferon, or Proof of Positive PPD and Chest X-Ray- SAM Search- Oregon State Background - CRIMS (facility run) Note : Ensure all credentialing items are completed before candidate submission to avoid declination.
