The Behavioral Health Consultant is part of the person-centered integrated care team and provides direct and indirect services to persons served in the medical services department.
This is a full-time, benefit eligible position working 40 hours weekly.
Brief Assessment and Behavioral Interventions
- Provide routine consultation (in person and remote) to primary care providers and patients on a wide range of health concerns including anxiety, depression, sleep, chronic pain, tobacco use, substance use, hypertension, stress management, diabetes, etc.
- Assist with administering and reviewing screening tools with patients and providers in the primary care setting to determine next steps for care including but not limited to PHQ-9, MDQ, SBIRT, GAD-7, EPDS, etc.
- Utilize motivational interviewing to assess patient's readiness for change and utilize stage matched interventions to assist with goal setting
- Partner with patients to identify barriers to medication / treatment adherence and chronic health management
- Work with the primary care team to identify care gaps and address quality metrics
- Provide culturally component care and utilize interpreting services as indicated
- Facilitate psycho-educational support groups on self-management of chronic health concerns
- Conduct brief crisis interventions and safety planning as needed
- Maintain client records, following agency and state regulations
- Maintain agreed upon level of productivity
- Demonstrate strong time management skills managing both scheduled and unscheduled appointments ("warm hand off")
Coordination of Care and Referral
Maintain routine communication with members of the healthcare team (PCP, nursing, therapist, nurse care manager, social worker, community health worker etc)Refer for further behavioral health services as neededCollaborate with Primary Care Health Care Home (PCHH) Nurse Care Managers (NCMs) for consultations within the programOther Responsibilities
Be knowledgeable of other human / social services for purposes of referral if the agency is unable to provide a particular service and maintain documentation of such referralsStay abreast of therapeutic techniques and best practices in the fieldInterface with other human / social services to provide wholistic services to persons servedBe knowledgeable of admission criteria for all programs as well as knowledge of Utilization Management criteria specific to insurance companiesProvide licensure supervision, if applicablePerform Utilization Review activities as assignedAttend all training as required by Compass Health Network, Accreditors and FundersMaster's degree in social work, marriage and family therapy, counseling, or psychology as recognized by the US Department of Education requiredDoctorate degree in Psychology as recognized by the US Department of Education preferredQUALIFICATION(S) : WORK EXPERIENCE / TRAINING / ADDITIONAL REQUIREMENTS
At least one (1) year of experience, under supervision, in treating problems related to mental illness in addition to behavioral issues as they relate to chronic medical conditions requiredWorking knowledge of DSM-5 preferredQUALIFICATION(S) : LICENSURE / CERTIFICATION
Must be licensed in Missouri under one of the following credentials :
