• Provide high-quality, compassionate social services to patients in their homes
• Collaborate with nurses, therapists, and physicians to ensure seamless care
• Support patients and families through illness, recovery, and life transitions What You'll Need
• Master's Degree in Social Work (from an accredited program)
• Licensed Social Worker (per Indiana state regulations)
• 2 years of experience in a healthcare setting (home care or hospice is a plus!)
• A heart for service, strong communication skills, and a professional, caring attitude
• Reliable transportation and ability to travel to patient homes Position Options
• Part-Time : Minimum 20 hours / week, includes benefits
