Medication History Technician - Pharmacy

Job Description

The Medication History Technician assists nurses and physicians in the collection of accurate medication history records for the ultimate purpose of physician medication reconciliation. The Medication History Technician works independently to collect medication records on new patient admissions both from the emergency departments as well as direct transfers to the hospital. The position is overseen by nurse, physicians, and pharmacists and is ultimately under the pharmacy department. Accurate medication histories may require interaction with the patient, family members, patient's pharmacy, and / or obtaining records from other facilities.
