Job Responsibilities
Home visits are a part of this roleAssigned to a PACE site to see all participants and ensure high-quality care.Supervise and manage Nurse Practitioners and assist with oversight of clinical staff.Conduct annual performance appraisals and provide feedback to clinical staff.Represent or serve as a Medical Director when needed.Perform annual wellness assessments and more frequent evaluations for high-risk participants.Provide urgent care services and assess participants with acute changes in condition.Conduct follow-up assessments post-hospitalization and coordinate care transitions.Review medical histories and formulate diagnostic lists for comprehensive care management.Ensure complete and accurate clinical documentation and participate in documentation improvement activities.Co-manage high-risk participants and address potential gaps in care with PCPs.Participate in interdisciplinary team meetings and represent CPCP as needed.Educate participants and families on disease processes, medication adherence, and self-management.Comply with HIPAA regulations and maintain the security of Protected Health Information.Conduct home visits as necessary and supervise home care services provided for community PCP participants.Provide emergency coverage for PACE center primary care providers and participate in on-call rotations.Perform other duties as assigned.Qualifications License Registration
Licensed to practice as a Nurse Practitioner in NY required.DEA License, and Medicare & Medicaid IDs are a must.Education
Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.Experience
Minimum of two years in clinical nursing practice in-home care, care management, or related field required.Effective oral and written communication and interpersonal skills required.High-level analytical and research skills.Ability to manage multiple projects and meet project deadlines.Basic Computer Skills in Microsoft Office.
