POSITION SUMMARY : The Community Relations Liaison, in conjunction with the other members of the Admission Team, is responsible for activities related to promoting the organization's services and expertise to referral sources, patients, families, and the community. The Liaison provides education to patients, families, caregivers, and facility staff regarding patient progress and other relevant areas to ensure high-quality care.
REPORTS TO : Director of Operations or Executive Director.
QUALIFICATIONS :
- Education : Bachelor's degree in a healthcare-related field preferred.
- Experience : 2+ years in hospice, palliative, home health, or similar fields preferred.
- Core Competencies : Knowledge of Medicare, Medicaid, and private insurance regulations; excellent communication skills; ability to observe and report on patient status; experience in marketing strategies.
- Other : Valid driver's license and current automobile insurance.
FUNCTIONS & RESPONSIBILITIES :
Collaborate with the admission team.Establish organizational presence within the community and at facilities.Assist with admission documentation and patient information.Respond to referrals and explain programs to patients and referral sources.Develop marketing plans with referral sources.Participate in staff meetings and documentation.Ensure compliance with policies, laws, and standards.Additional Details :
