The MDS Coordinator plays a vital role in ensuring accurate resident assessments, individualized care planning, and regulatory compliance. This position supports residents, families, and staff by coordinating care plans, monitoring documentation, and fostering a collaborative care environment.
✨ Job Summary
As the MDS Coordinator, you will :
Ensure resident care plans are accurate and up to date.
Oversee documentation to align with care plans and services provided.
Coordinate timely completion of Comprehensive Resident Assessments.
Communicate resident conditions with families and the care team.
Report to the Administrator regarding compliance and care planning.
✅ Qualifications
Current Illinois license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
Ability to read, write, and communicate clearly in English.
AANAC certification preferred.
Compassion, patience, and understanding for the elderly.
Job Requirements
Strong knowledge of nursing services, rehabilitation, geriatric care, and MDS / EMR systems.
Excellent organizational, managerial, and interpersonal skills.
Skilled in care planning, staff training, and EMR / documentation oversight.
Knowledge of JCAHO, OBRA, IDPH, and HFS standards.
Physically and mentally able to perform routine job duties.
Key Responsibilities
Coordinate timely MDS completion for admissions, readmissions, quarterly / annual reviews, significant changes, and PPS / insurance reimbursement.
Maintain a calendar of MDS assessments and Care Plan Conferences.
Ensure completion of MDS sections by appropriate staff.
Provide re-education for staff to maintain documentation compliance.
Review and update diagnosis coding (ICD-9 / ICD-10) with physicians quarterly.
Conduct weekly MDS pre-planning and Medicare meetings.
Collaborate with the interdisciplinary team to resolve MDS-related issues.
