CASE COORDINATOR
Job Category : Behavioral Interventionist (BI) Program
Job Status : Full-time
Reports to : BI Program Manager
Job Summary : The Case Coordinator provides supervision to frontline BIstaff and case management to families served in the BI Program.
Responsibilities
Case Management
- Family satisfaction : ensure families are given proper support, advocacy, resources, communication
- Complete required assessments with families, adapting to the needs of the client and family
- Attend court appointments + other professional family-based meetings
- Matching BIs to homes, resolving issues, attend team meetings
- Review Incident Reports, follow-up with family, if needed
- Facilitate Referral Triage, Intake & Meet & Greets for each family
- Submit approved Recommendation & Approval Letters for services
- Create Behavioral Plan for each client
- Provide updated communication during the Admissions & Hiring processes
- Complete Quarterly Meetings &Bi-annual Assessments with each family
- Write / submit renewals
- Update SAM, electronic health record, & profiles
- End of Month Reports
- Case Closure & Aftercare Follow-up meetings,
assessments, and documentation

MA / MS (or BA / BS +2 years working in non-profit social service agency) in social work, psychology, public administration, or related human services fieldPrevious experience working with at-risk populationsFamiliarity with the child welfare systemResourcefulness in setting priorities, proposing new ways of creating efficiencies, guiding investment in people & systemsUnderstanding of program development, implementation & evaluationOrganizational & planning skillsCommitment to principles of support & advocacy for foster & adoptive familiesStrong verbal & written communication skillsLeadership, team management, interpersonal skills; quickly establish rapport & build trustAdvocate for positive outcomes within multidisciplinary, diverse & dynamic professional teamsFlexible, self-starter; multi-tasker, detail-oriented can meet deadlinesModel ethical / professional behaviorExperience working with marginalized communities with a nuanced understanding of systemic oppressionsCompensation & Benefits
