ALAMEDA FAMILY SERVICES
Alameda Family Services (AFS) will provide a master's level clinical social worker who will work directly with the City of Alameda's Housing and Human Services Division (HHS) partnering with The Alameda City Library to deliver best practice social work interventions, informal mental health support to Library patrons, consultation to library staff, and linkage to services, including to City Case Managers. The clinician will provide 30 hours of on-site support per week for a term of 12 months.

Programs include : Early Head Start, Head Start, Family Support Services, School Based Services, and Behavioral Health Care Services (Counseling and an APA- accredited Narrative Therapy Training Program). Crisis Services Division
This division encompasses clinical consultation and follow up clinical case management services for the Alameda CARE Team mobile crisis response program as well as co-located therapists and clinical case managers supporting the mental health and well-being of individuals in Alameda.
OVERALL RESPONSIBILITIES :
The Clinical Social Worker will provide trauma-informed, and culturally sensitive social work and informal counseling to library patrons. The Clinical Social Worker will conduct Social Determinants of Health (SDOH) assessments and find matching, appropriate linkages to City and County services provided to patrons who are self-referred, or library staff referred. Linkages include a warm handoff for patrons. The clinical social worker will develop relationships with hard-to-engage patrons experiencing mental health challenges. They will provide best practice clinical consultations and case conferencing, as needed to support library staff and social service providers' ability to support patrons with needs.
