We're Hiring : RN Case Manager!
We are seeking a compassionate and organized RN Case Manager to join our Mobile Care Unit team based in Rockville, MD . This unique hybrid position includes both field-based patient care and remote case management , serving diverse communities with a focus on accessibility and preventative health. The ideal candidate is comfortable working independently, has strong assessment and communication skills, and is passionate about community-based healthcare.

What You'll Do :
Field Work (3 Days / Week) :
- Travel independently to designated mobile care sites where the RV-based clinic is stationed.
- Conduct in-person assessments, health screenings, and basic nursing care in collaboration with the mobile care team.
- Support chronic disease management, vaccinations, patient education, and medication administration.
- Provide care coordination and communicate with providers and community partners as needed.
- Maintain accurate and timely patient records and documentation on-site.
Remote Work (2 Days / Week) :
Perform telephone and virtual case management and follow-up assessments.Coordinate care plans and referrals for patients needing additional services.Monitor patient progress, compliance, and outcomes.Support patients with health coaching and education via telehealth platforms.Other Duties :
Participate in community outreach and wellness initiatives.Be available for 1–2 Saturdays per month to support weekend mobile unit events or health fairs.What We're Looking For :
Licensure : Active Registered Nurse (RN) license in the State of Maryland .Education : Associate Degree in Nursing (ASN) required; BSN preferred.Certifications :BLS (Basic Life Support) – Current and valid.PPD (Tuberculosis test) – Current documentation required.Criminal background check, drug testing requiredExperience :
Minimum 1 year of nursing experience in community health, primary care, or case management preferred.Experience with underserved populations is a plus.Skills :
Strong clinical assessment, organizational, and communication skills.Comfort working independently in non-traditional care settings.Basic proficiency with telehealth platforms and electronic health records (EHR).Additional Requirements :
Must have a valid driver’s license and reliable transportation.Flexibility to adapt to changing schedules and mobile unit locations.Working Conditions :
