Key Responsibilities : As an LPN, you will play a crucial role in the clinical care of our residents : Perform daily documentation of resident care, status, and changes in condition accurately and completely in compliance with facility policy and regulations. Transcribe physicians’ orders to MARs (Medication Administration Records) and treatment sheets, ensuring meticulous record-keeping. Notify the facility pharmacy promptly of new or changed orders to ensure continuity of care. Prepare and administer medications and perform treatments as ordered by the physician, strictly within the LPN scope of practice in Wisconsin. Monitor and assess residents' physical and emotional well-being throughout your shift, reporting any significant changes to the Registered Nurse or Director of Nursing immediately. Supervise Certified Nursing Assistants (CNAs) on your unit to ensure the delivery of high-quality personal care. Participate actively in the interdisciplinary care team to help develop and implement individualized care plans. All other duties as assigned by the Unit Manager or Director of Nursing. Qualifications : Must possess an Active and unrestricted Licensed Practical Nurse (LPN) License within the state of Wisconsin. Strong organizational, communication, and basic computer skills are essential for accurate charting. Previous experience in a long-term care or skilled nursing setting is a plus, but highly motivated new graduates are encouraged to apply!
