Position Summary :
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long-term care facility under the supervision of a Registered Nurse or Physician. The LPN is responsible for delivering safe, compassionate, and effective nursing care while supporting residents’ well-being and maintaining compliance with regulatory standards.
Key Responsibilities :
- Provide direct nursing care to residents according to care plans and physicians’ orders.
- Administer medications and treatments accurately and in a timely manner.
- Monitor and document residents’ health status , reporting any significant changes to the RN or physician.
- Assist with resident assessments and the development of individualized care plans.
- Supervise and direct CNAs and other nursing assistants in delivering daily care.
- Ensure all documentation is complete, accurate, and in compliance with facility policies and state / federal regulations.
- Respond to emergencies and follow proper protocols.
- Support residents and families with health education and emotional support.
- Maintain compliance with infection control , safety , and HIPAA standards.
- Participate in interdisciplinary team meetings , audits, and in-service training.
Qualifications :
Valid LPN license in the state of employmentGraduate of an accredited Licensed Practical Nursing programCPR / BLS certification (required or able to obtain upon hire)At least 1 year of nursing experience (long-term care preferred, but not required)Ability to communicate effectively and compassionately with residents, families, and team membersFamiliarity with electronic medical records (EMR) systems preferredStrong critical thinking, organizational, and time-management skillsPhysical Requirements :
Ability to lift / move up to 50 lbsFrequent standing, walking, bending, and reachingAbility to respond quickly to resident needs or emergencies
