Licensed Behavioral Health Case Manager

A company is looking for a Case Manager in Behavioral Health (Eating Disorder) - remote (PA / NJ / DE).

Key Responsibilities :

Engages members telephonically to identify barriers to care related to Social Determinants of Health (SDOH)

Facilitates the development of care plans by identifying members' strengths, needs, and goals

Acts as a liaison within the member's health care team, especially in complex behavioral health situations

Qualifications :

Active PA Registered Nurse (BCBA preferred), LCSW, LSW, or LPC required

3-5 years post-licensure experience in an integrated care or healthcare setting

Strong understanding of Social Determinants of Health and Motivational Interviewing techniques

Ability to work independently and resolve complex behavioral, medical, and psychosocial issues

Exceptional assessment, engagement, and organizational skills
