As part of a support coordination team who will manage members enrolled in the Employment and Community First CHOICES (ECF CHOICES) program, the ECF CHOICES Support Coordinator will be the primary support coordinator for a panel of members with intellectual or developmental disabilities. Support coordination activities will focus on supporting member's medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care.

This is a home-based position with field responsibilities approximately 75% of the time. Travel is expected in the following counties - Sumner Co and surrounding areas in TN.

Required Qualifications :

Meet one of the following :

4-year or higher degree in social work, nursing or other health care profession plus 3+ years of direct experience

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.27 to $50.48 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
