Key Responsibilities
Leads system-wide population health projects and initiatives, developing implementation plans and operational workflows
Provides subject matter expertise on value-based quality measures and enhances contract performance through data validation
Directs a team of Population Health Coordinators to support quality outcomes and performance within value-based care programs
Required Qualifications, Training, and Education
Current RN License in the state of work or covered by compact (required)
BSN or Bachelor's in Healthcare or related field (required)
7 years of business / healthcare experience in leadership, quality, and project management (required)
Experience with managed security service providers, NCQA, and CMS (required)
EPIC training preferred
