Location : Must reside in Arkansas or a border city

Pay Rate : $23 / hr

Shift : Monday - Friday, 8 : 00 AM - 5 : 00 PM

Dean's Professional Services is actively seeking a Care Coordinator II to support care management activities and ensure continuity of care and member satisfaction. This role works closely with healthcare providers, partners, and members - performing outreach telephonically and through home visits while maintaining accurate care documentation.

Responsibilities

Coordinate care activities according to the care / service plan in collaboration with healthcare providers, community partners, and members / caregivers.

Serve as a support resource for member and provider inquiries, requests, or concerns related to care plans.

Communicate with care managers, practitioners, and others to facilitate member services and ensure continuity of care.

Perform service assessments / screenings and document member care needs as required.

Maintain accurate member records in compliance with state and regulatory guidelines.

Identify needs and make referrals to Care Managers, Disease Managers, or community-based organizations.

Provide education on benefits and available resources.

Perform other related duties as assigned.

Qualifications

High School diploma or GED required.

1-2 years of related experience preferred.

Valid driver's license required.

Travel : Up to 30% required.
