Under the supervision of the Tuberculosis (TB) Team Leader and TB Unit Manager, provides TB services for clients within an assigned area. Plans, implements and evaluates direct nursing care and coordination of services to individuals and families receiving TB services within an assigned area.
This position requires role-based access to South Carolina Infectious Disease & Outbreak Reporting Network (SCION), Client Automated Record & Encounter System (CARES), and the Electronic Health Records (EHR) for surveillance activities, case reporting, verification of diagnosis and treatment status, and documentation of patient care activities. Leadership / Self Development : Member of an interdisciplinary team with team case conferences and cross coverage in time of staffing absences and / or increase case management. Ongoing training opportunities. Helps facilitate orientation of new TB staff as directed. Case Management : Provides holistic care by assessing patient / family resources, community resources, barriers to care, and utilization of services to determine an appropriate plan of care for the patient with Latent TB Infection (LTBI) or TB disease. Ongoing evaluation of care during treatment with the patient, TB Team, and the patient's primary care provider. Conducts contact investigations.
Completes physical assessments and health histories of patients according to TB program guidelines. Obtains necessary lab work including, but not limited to, complete blood count (CBC), liver function tests, Interferon-Gamma Release Assay (IGRAs), H uman Immunodeficiency Viruses ( HIV) tests, and sputum specimens. Provides treatment and / or administration of medication under a physician's order or standing order. Provides counseling and education regarding TB testing, evaluation, and treatment for Latent TB Infection (LTBI) or TB disease to patients and external partners. Administrative Duties : Participates in Continuous Quality Improvement (CQI) activities such as record reviews, program evaluations, team meetings, etc. Maintains records and reports in accordance with agency standards including documentation in the patient record (EHR), Personnel Cost Accounting System (PCAS), weekly reports, etc. Emergency Response / Nursing Practice : Assists with shelter operations, maintains supportive nursing practice competencies to include, but not limited to, intramuscular, subcutaneous, and intradermal best practice injection administration, oral medication administration, and client emergency response including First Aid, Cardiopulmonary Resuscitation ( CPR) and Automated External Defibrillator ( AED), and be willing and available to assist during times of public health emergency and outbreak response efforts.
Minimum And Additional Requirements
State Minimum Requirements : Graduation from an accredited school of nursing. Licensure as a registered nurse by the State Board of Nursing for South Carolina.
Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.
Agency Additional Requirements : Licensure as a compact state registered nurse, must obtain South Carolina Board of Nursing (SCBON) endorsement upon hire. Employee will obtain Basic Life Support (BLS) / Cardiopulmonary Resuscitation (CPR) / First Aid certification within thirty (30) days of hire or provide documentation of satisfactory course completion prior to employment. Must have a valid driver's license. May be required to use personal vehicle. Must be able to make home visits. May need to use stairs to enter client homes and / or encounter domestic animals. Significant travel and / or overnight within the state may be required.
Must be able to bend, stoop, and lift thirty (30) pounds. Must be able to pass a fit-test and be able to use appropriate respiratory protective equipment; Employee will be in contact with an infectious or potentially infectious tuberculosis (TB) case / suspect. Must participate in disaster preparedness during hazardous weather or declaration of an emergency, subject to duty and / or call on a 24-hour basis. May require flexible work hours.

Preferred Qualifications
A bachelor's in science of nursing. At least one (1) year of nursing experience. At least one (1) year of home-health or community health experience. Bi-lingual (Spanish / English) helpful. Tuberculosis case management experience. Experience with Electronic Health Record (EHR); technology databases, applications, and record-keeping.

DRIVING RECORD : If this position requires the applicant to possess a valid driver’s license to operate a state vehicle or personal vehicle, any applicant being considered in the final stages of selection for the position will be required to provide a certified copy of a 10-year driving record.
IMMUNIZATION REQUIREMENTS : All new employees who are healthcare providers with job-specific exposure risk are required to provide documentation of immunity or be immunized against measles, mumps, rubella, pertussis, varicella, and Hepatitis B prior to beginning employment.
