Must Have Skills / Experience : Highly organized, work in fast paced environment, adaptable, effective written and verbal communication via in-person, telephone and email communication, basic skills excel, work, outlook, carry out duties independently, teamwork, uphold confidentiality

Preferred Skillset / Experience : Case Management experience, inpatient rehabilitation experience, hospital experience

Common Diagnosis / Treatment : Stroke, Brain Injury, Amputation, Multiple Trauma Injury, Spinal Cord Injury

Personality Fit : Responsible, Outgoing, High Integrity, easily makes connections, tolerant
