Contract : 13 weeks (possible extension)
Shift : 8hour day shift (40 hours a week) Monday-Friday 8 : 00am-4 : 30pm

Job Description :
We are seeking a compassionate and experienced Medical Social Worker (Licensed Clinical Social Worker - LCSW) to join our client's hospital care management team. This role is responsible for providing psychosocial assessments, discharge planning, crisis intervention, and supportive counseling to patients and their families while ensuring seamless coordination of post-acute services.
Key Responsibilities :
Discharge Planning & Coordination :
- Assess patients for discharge needs and complete evaluations per departmental policy.
- Collaborate with physicians and interdisciplinary teams to create and document post-discharge care plans.
- Act as a liaison to connect patients with community resources and enhance safe, timely discharges.
- Attend and participate in patient care conferences to support discharge planning decisions.
Counseling & Psychosocial Support :
Provide crisis intervention and adjustment counseling.Evaluate support systems, financial challenges, and family dynamics affecting patient care.Deliver age-appropriate counseling and education to patients, families, and significant others.Facilitate support groups and provide resource referrals for ongoing mental health services.Documentation & Compliance :
Maintain thorough documentation of psychosocial assessments, interventions, and discharge plans.Ensure records meet all licensure and third-party payer requirements.Complete required audits, statistical records, and data entry per policy.Education & Team Involvement :
Participate in continuous education and maintain CEU documentation per NASW standards.Conduct in-services and support unit-based training and education initiatives.Serve as a key member of the care management team and collaborate closely with case managers and healthcare providers.Process Improvement :
Contribute to departmental performance improvement efforts.Suggest innovative practices to enhance efficiency and patient outcomes.Qualifications :
Education & Licensure :
Master’s Degree in Social Work (MSW) from a CSWE-accredited institution.Current licensure as a Licensed Clinical Social Worker (LCSW).Experience :
Hospital-based field experience required.Demonstrated competence in discharge planning and clinical counseling.Skills & Abilities :
Knowledge of community resources, behavioral health practices, and healthcare coordination.Ability to manage multiple priorities and respond effectively to crisis situations.Strong interpersonal, communication, and documentation skills.Must adhere to NASW code of ethics and professional standards.
