GENERAL SUMMARY :
Provides in home Counseling services to Treatment Foster Care children and Foster families. Provides ongoing case support, coordination and advocacy services to ensure a safe and stable environment for children in out of home care. Ensures homes are licensed and maintained according to Wisconsin state statutes DCF 54, 56 & 12 including any required training.
ESSENTIAL FUNCTIONS :
- Reviews referrals for TFC placement and assists with making appropriate matches.
- Conducts initial licensing visits to perspective TFC parent to ensure initial compliance with licensing standards.
- Complete applicable licensing paper work, reports and documentation in a timely manner for final approval of Program Manager.
- Maintains program identified caseload of TFC children and foster homes (usually 8-11 for full time and 4-7 for part time).
- Provide in home support consultation, one to one training, counseling and support services at minimum bi-weekly, which may include individual and family consultation and meetings.
- Crisis intervention, including responding to specific needs, after hour responsibilities based on team coverage needs.
- Supports the goals of the agency mission.
- Inputs data and thoroughly documents all interactions in required agency systems for analysis and review.
- Assist in training and facilitating sessions and support groups with TFC parents and respite providers.
- Attend and participates in relevant court and / or school appointments, medication reviews, team meetings and represent the agency professionally and positively at all times.
- Provides discharge planning and aftercare services for the child in care.
- Facilitate TFC treatment planning and reviews.
- Maintains client / parent records as mandated by DCF 56 and agency protocols.
- Provides referrals and advocates for child and foster parent as needed.
- Writes reports related to child’s treatment (court reports, client assessments, progress notes, treatment plans, etc.)
- Collaborate with child’s treatment providers and other professionals.
- All other duties as assigned.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED :
Bachelor’s degree in Social Work or related field required, Master’s degree preferred.If Bachelor’s degree at least three years’ experience working with youth in Out of Home placement settings or relevant environments.If Master’s Degree, at least one year experience in Social Services and working with youth in Out of Home placements.Certification in Social Work required, licensing is preferred.Experience with SED clients preferred.Ability to display cultural competence by responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions and other diversity factors in a manner that recognizes, affirms and values the worth of individuals, families and communities and protects and preserves the dignity of each.Proficient in computers skills with the ability to use Internet, Microsoft office products, email, virtual platforms, etc.Ability to express self verbally and in written formats.Valid Wisconsin Regular Driver’s License, clear of major moving violations.Ability to provide proof of personal auto liability insuranceAbility to work a flexible schedule, some evenings and weekends required weekly.Group facilitation experience a plus.Ability to work cooperatively using a team approach.Ability to assess and plan for permanency placements for foster childrenAbility to assist foster parents, biological parents and child in obtaining reunification whenever possible.Knowledge of basic computer applications (Microsoft office).Ability to utilize personal vehicle to conduct / complete agency tasks.Ability to communicate effectively with clients and other staffWORKING CONDITIONS :
