Job Title : Nurse Case Manager – Inpatient Care (Med-Surg)\n\nLocation : Las Vegas, NV\n\nSchedule : Full-Time | Monday–Friday, 8am–5pm (flexibility for weekends / on-call)\n\nAbout the Role\n\nThe Nurse Case Manager plays a key role in providing a coordinated, collaborative, and multidisciplinary approach to patient care across the health care continuum at the hospital. This position focuses on inpatient discharge planning and utilization review, ensuring patients receive timely, appropriate, and cost-effective care.\n\nAs an advanced clinical resource, the Nurse Case Manager supports patients, families, staff, and physicians in delivering high-quality, patient-centered care.\n\nKey Responsibilities\n
- Perform case management functions including assessment, care coordination, discharge planning, and utilization review for inpatient Med-Surg units.\n
- Collaborate with physicians, nurses, and ancillary staff to ensure smooth transitions of care.\n
- Evaluate medical necessity, level of care, and appropriate discharge disposition.\n
- Use Epic EMR to document assessments, care plans, and communications.\n
- Support high patient census environments with flexibility and critical thinking.\n\nRequired Qualifications\n
- Graduation from an accredited school of nursing.\n
- Minimum 3 years of acute care nursing experience (sub-acute experience accepted only from Kindred or Risin Specialty).\n
- At least 1 year of experience in Case Management, Discharge Planning, or Utilization Review.\n
- Current RN license in the State of Nevada (or ability to obtain).\n
- Proficiency with Epic EMR preferred.\n
- Strong communication, collaboration, and organizational skills.\n\nSchedule\n
- Monday–Friday, 8 : 00am–5 : 00pm\n
- Flexibility required for shifts, weekends, and on-call coverage as needed\n
- Once oriented, a set schedule will be provided\n\nCompensation & Benefits\n
