Location : This position is based out of LifeWorks NW's Beaverton site.

Pay : $23.92 starting hourly pay.

Overview

The Substance Use Disorder Counselor, On Call position provides substance use disorder treatment services in an outpatient setting for adults. Responsibilities include assessment, treatment planning, provision of counseling (individual, family, and group), urinalysis, consultation with care providers, and coordination of multiple service providers for the assigned client caseload, with timely and accurate records.

Essential Responsibilities

• Client evaluation including initial intake interview, formulating a substance use diagnosis, identifying appropriate treatment services, notifying client and involved parties of diagnosis and initial treatment recommendations, completing written evaluation, and obtaining necessary authorizations for disclosures.
• Gather information about individuals using interviews, case histories, observations, urinalysis, and other assessment methods; observe and evaluate client performance, behavior, social development, and physical health.
• Analyze data to identify symptoms and determine an ASAM PPC-2R level of care.
• Identify and diagnose substance use disorders using information from interviews, tests, records, and references; address legal issues, assist with hearings, provide testimony for custody cases; screen for mental health, trauma, and other issues and refer as needed.
• Provide referral services to clients and families (e.g., residential, health care, mental health, employment, housing, recovery support); maintain awareness of community resources.
• Develop therapeutic ISSP plans based on client needs; ensure plans are evidence-based, measurable, and client-driven; complete timely follow-ups.
• Coordinate post-treatment plans with clients, significant others, and treatment teams to maintain recovery.
• Counsel individuals or families using evidence-based practices; help clients define goals and develop action plans; teach coping skills.
• Facilitate or co-facilitate group treatment services using evidence-based models; monitor fidelity and outcomes.
• Service coordination as needed; collaborate with physicians, staff, families, and referents; maintain notes and progress reports.
• Evaluate effectiveness of counseling and diagnoses; update ISSP and Behavioral Health Assessments regularly.
• Document and maintain client records in the electronic health system within 24 hours of meetings; ensure confidentiality and compliance.
• Provide training to staff, caregivers, and families on substance use issues; provide crisis intervention; may serve as Clinician of the Day.
• Administer and report urinalyses with accuracy; travel between sites or in the community as needed; some evenings may be required.
