Job Description :

Hospice RN Case Manager

At Elara Caring, we believe hospice care is about living well —not giving up. As a Hospice RN Case Manager , you'll bring compassionate, patient-centered care directly into people's homes, helping patients spend their final days with dignity, comfort, and peace.

Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM

What You'll Do

Provide high-quality nursing care in patient homes

Complete assessments and create personalized care plans

Monitor and respond to changes in patient conditions

Partner with physicians and the interdisciplinary team to coordinate care

Educate patients and families on comfort care, medications, and disease progression

Support families with compassion through the end-of-life journey

Ensure compliance with physician orders and care standards

What You'll Bring

Graduate of an accredited nursing program (Diploma, ASN, or BSN)

Current RN license in Michigan

1+ year of nursing experience (hospice or home health preferred)

Strong clinical judgment, communication, and organizational skills

Reliable transportation for home visits within Oakland County

Ability to lift up to 50 lbs and meet the physical needs of the role
