Join Our Team as a Home Health Physical Therapist!

As a Physical Therapist in Home Health , you will play a vital role in assessing and evaluating patient needs related to mobility, endurance, pain, and other medical conditions. Your expertise will help shape treatment plans that truly make a difference in our patients' lives!

Compensation : Pay is negotiable, ranging from $60,847 - $110,406 annually based on experience.

Your Responsibilities :

• Evaluate patients' functional status and physical therapy needs, collaborating with physicians to develop effective therapy plans.
• Provide services within your scope of practice, ensuring coordination with the healthcare team.
• Monitor and report patient responses to treatment, keeping the nurse supervisor and physician informed of any changes.
• Utilize evidence-based techniques to relieve pain and restore function, helping patients achieve their maximum potential.
• Educate patients, families, and healthcare team members about the plan of care and progress toward goals.
• Conduct patient visits across the service area, adapting to the needs of our patients.
• Document progress notes and evaluations in accordance with LHC policy.
• Participate in staff conferences and committees to foster a collaborative work environment.
• Supervise home health aides and Physical Therapist Assistants as per LHC policy and state regulations.
• Prepare written discharge summaries and condition reports every 60 days.
• Provide in-service education as requested by agency leadership.
• Adhere to LHC Therapy Standards of Practice in all patient interactions.
• Engage in continuous education to maintain and enhance your clinical skills and licensure.
• Perform other duties as assigned to support our mission.
