Salary Range : $16.50- $21.60

Job Responsibilities :
1. Be present in the patient's / client's home at specified hours to participate in requested support as well as observed needs, including, but not limited to.
2. Playing games

4. Talking and listening

6. Companionship and friendship

8. Meal preparation
9. Light gardening
10. Light housekeeping
11. Provide emotional support, friendship and encouragement.
12. Drive / escort clients to medical appointments, errands, shopping, church, airport or train station and outings as specified in the support plan.
13. Meal preparation (shopping, preparing, cooking, serving, clean-up).
14. Light House cleaning (changing bed linen, vacuum, clean shower, tub, toilet, sinks, counter tops, refrigerators, straighten, dust and other similar tasks).
15. Laundry (washing, drying, folding, ironing, hanging).
16. Assist with paperwork and organize mail.
17. Pet care (walk, clean-up after coordinate vet and groomer visits).
18. Ensure client's safety and security in the home or on outings.
19. Follow the written plan of support.
20. Report changes in the patient's physical and / or cognitive status, and / or changes in the client's environment to the Delegating Nurse or their designee.
21. Complete flow sheets and time sheets during each service visit, and submit flow sheets and time sheets to the office in accordance with the organization policies and procedures.
22. Participate in team meetings by providing input and making suggestions.
23. Will work under the supervision of the Delegating Nurse or their designee.
24. Will work with the scheduler to ensure coverage of Clients as needed.
25. Direct supervisor is the Clinical Director of Home Care or designee.
26. Will independently develop a schedule of hours needed to meet the client's needs on a routine or as needed basis.
27. Will schedule hours with the client and maintain records in accordance with all state, federal and billing agencies.
28. Will update the Staffing Coordinator or designee of hours worked with the client weekly and any changes as they occur in the clients' schedule.
29. Will use the Clear Care app for clocking in and out of all client shifts and maintain time sheets to reflect hours worked with client(s).
30. Will update the Clinical Director of Home Care or designee of all changes in condition with the client.
31. Must have the ability to maintain confidentiality of all client information and assure client's rights and privacy is protected at all times.
32. Must have the ability to accurately plan work routine and complete assigned duties.
33. Must be able to work in an environment that includes pets and pet care.
34. Attend Broadmead orientation, as required.
35. Complete Relias in-service training, as required.
36. Attend staff meetings.
37. Help train colleagues.
38. Must maintain a professional, neat, clean appearance and follow the dress code policy.
39. Must be able to clock in and out at the time clock or use ESS mobile application (supervisor approval required).
