RN Case Manager / Medallion

Provides services to clients in accordance with the State Nurse Practice Act, agency policy, and accepted professional standards of practice. The Registered Nurse Case Manager provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates / documents observations and assessments. Responsible for the outcome of applicable OBQI / QA studies. Reviews all processes on a yearly basis to assure those processes are current and within or exceeding guidelines. Requirements

• Current licensure as an RN in the state of Montana
• Current CPR certification Physical Requirements
• Ability to move independently or with reasonable accommodation within the facility and community.
• Applicant must be physically and mentally able to perform all requirements of the job.
• Must have a reliable vehicle.
• All positions require the completion of a background check. Acknowledging or having criminal convictions does not constitute an automatic bar to employment.
• Applicant must pass a DMV check. Experience
• 0-2 years
• Experience in geriatrics care helpful
• Associate's degree
