• Location : Bluffton, SC, and surrounding areas\n
• Company : Sunbelt Staffing in partnership with a Nationwide Home Health Agency\n
• Contract Length : 13 Weeks (with possible extension upon successful start and completion)\n
• Hours : Full-time, 40 hours per week, no guaranteed hours\n
• Monday-Friday, 8 am-5 pm, with occasional weekends and on-call duty (typically one weekend per month on-call)\n
• Compensation : Negotiable with client, based on experience and territory budget; paid hourly - not by visit; blended pay packages available for travelers; weekly pay\n
• Productivity : 30-32 per week, up to 35 visits per week\n
• Mileage Reimbursement : 0.38c per mile from patient home to home, 150-250 miles per week expected\n

• Conduct patient assessments using OASIS, ensuring accurate and timely documentation.\n
• Develop and implement personalized care plans in collaboration with physicians and other healthcare professionals.\n
• Provide skilled nursing care, including wound care, medication administration, and monitoring of vital signs.\n
• Educate patients and their families on care plans, medication management, and other health-related topics.\n
• Maintain accurate patient records in Homecare Homebase (HCHB) and ensure compliance with agency standards.\n
• Travel to patient homes within the area, utilizing reliable transportation.\n
• Participate in case conferences and team meetings to ensure coordinated and comprehensive care.\n\nRequirements\n
• Active Registered Nurse (RN) license in the state of South Carolina.\n
• Minimum of 1 year of home health experience.\n
• Proficiency in OASIS and Homecare Homebase (HCHB).\n
• Current American Heart Association or American Red Cross Basic Life Support (BLS) certification.\n
• Reliable transportation with a valid driver's license.\n
